Μόλις κυκλοφόρησε και στην Ελλάδα από την Sanofi Pasteur MSD
εμβόλιο για την πρόληψη του Έρπητα Ζωστήρα κ της Μεθερπητικής Νευραλγίας. Είναι το μοναδικό στο είδος του για την ώρα.

Λέγεται ZOSTAVAX και αποτελείται από ζώντες ιούς.
Πρόκειται για ζων εξασθενημένο εμβόλιο που ενισχύει την ειδική κυτταρική ανοσία στον ιό της ανεμευλογιάς η οποία ανοσία μειώνεται συνήθως με την ηλικία.
Το 95% των ενηλίκων έχουν προσβληθεί κάποτε από ανεμευλογιά και είναι αυτοί που κινδυνεύουν, καθώς παιρνούν τα χρόνια, να νοσήσουν από Έρπητα Ζωστήρα καθώς μειώνεται με την ηλικία η ανοσία τους ενώ ταυτόχρονα ο ιός της ανεμευλογιάς υπάρχει, στα άτομα αυτά, σε λανθάνουσα κατάσταση.

Το ZOSTAVAX μειώνει στατιστικώς σημαντικά τη συχνότητα εμφάνισης έρπητα ζωστήρα και μεθερπητικής νευραλγίας.

Δίνεται σαν μία δόση εφάπαξ και χορηγείται υποδόρια ή ενδομυϊκά στον δελτοειδή μυ.

Μπορεί να γίνει ταυτόχρονα με το αντιγριππικό (στο ένα μπράτσο το ένα στο άλλο μπράτσο το άλλο) αλλά όχι ταυτόχρονα με το Pneumo 23.

Στις 09/09/2014 χορηγείται δωρεάν (συμμετοχή 0%) σε όλους όσους είναι άνω των 60 ετών, αν και έχει ένδειξη για άνω των 50 ετών. Αυτοί που είναι όμως μεταξύ 50 και 60 ετών πληρώνουν ολόκληρη την Λιανική Τιμή.

After 10 years, vaccination lost most of its power.

Short-term follow-up of participants in placebo-controlled studies of the herpes zoster (HZ) vaccine confirmed that the vaccine was efficacious for at least 5 years (NEJM JW Gen Med Dec 1 2012 and Clin Infect Dis 2012; 55:1320). Now, long-term follow-up of some original vaccine recipients, including placebo-group members who were vaccinated when the studies were completed, has allowed investigators to estimate longer-term efficacy.

Almost 7000 vaccine recipients (median age, 74) were monitored for about 4 years after the original studies ended, most through year 10 after vaccination; of 347 suspected cases of HZ in this cohort, 263 were confirmed. Historical controls were used to assess several measures of vaccine efficacy. Efficacy against HZ incidence fell from 46% in year 7 to 14% in year 10 and was negligible among the 1470 participants who were followed for an 11th year. Efficacies against postherpetic neuralgia and against a composite “burden of illness” score also were negligible by year 11. Overall estimates of vaccine efficacy between 5 and 12 years after vaccination were 21% against HZ incidence, 35% against postherpetic neuralgia, and 37% against “burden of illness.”

COMMENT

These numbers suggest that a single dose of herpes zoster vaccine administered at age 60 is unlikely to confer protection for the duration of a person's life. The authors caution that absence of a true placebo group precludes drawing any firm conclusions from this study's data. However, an editorialist cautiously suggests “rethinking public health recommendations” and possibly endorsing a second dose of vaccine about 8 years after the first.

Zoster Vaccine at Age 50?

For healthy patients, the expense is unjustified.

The zoster vaccine is licensed for use in older adults (age, ≥50), but most guidelines endorse its use only starting at age 60, when the incidence of herpes zoster (HZ) begins to climb. To address this discrepancy, researchers assembled a cost-effectiveness analysis of vaccine use among healthy immunocompetent 50- to 59-year-olds. Variables included age-specific rates of HZ, rates and expenses of disease complications, side effects and expense of vaccination, and expected duration of vaccine-related immunity (efficacy declines by an estimated 50% after 5–6 years; NEJM JW Gen Med Apr 1 2015 and Clin Infect Dis 2015; 60:900).

The final model predicted that for every 1000 patients vaccinated at age 50, 25 cases of HZ and 1 case of postherpetic neuralgia would be prevented, at an estimated cost of more than US$323,000 per quality-adjusted life-year saved. This amount far exceeds accepted thresholds for good value in healthcare. Cost-effectiveness estimates improved with increasing age for vaccination at ages 51 to 59, but they continued to be inferior to those for vaccination at age ≥60.

Comment

The duration of zoster vaccine effectiveness appears to be short enough that earlier is not better when it comes to protecting older adults. This analysis joins others in affirming age 60 as the sensible time to begin vaccinating healthy immunocompetent adults.

Μίλησα στις 23/05/2018 με την εταιρεία που διαθέτει το Zostavax στην αγορά: Το Zostavax μπορεί να το χορηγήσει κανείς και σε άτομα που έχουν περάσει ανεμευλογιά αλλά και σε άτομα που δεν έχουν περάσει ανεμευλογιά.

Ο ΕΟΠΥΥ καλύπτει το εμβόλιο (με συμμετοχή του ασφαλισμένου 0%) από την ηλικία των 60 ετών και πάνω. Για όσους θέλουν να το κάνουν πριν από την ηλικία των 60 ετών πρέπει να το πληρώσουν από την τσέπη τους.
