Μιας και η κολχικίνη θα μας απασχολήσει από δω και πέρα καλό είναι να γνωρίζουμε πως πέρα από την αντένδειξη χορήγησης σε άτομα που λαμβάνουν μακρολιδη, κυκλοσπορινη και βεραπαμιλη (που αναφέρονται στις επίσημες οδηγίες του ΕΟΔΥ) σύμφωνα με το Medscape υπάρχει αντένδειξη στη συγχορήγηση και με άλλα κοινά φάρμακα όπως οι στατινες, οι φιμπράτες, η αμιοδαρωνη, η καρβεδιλολη, η διλτιαζεμη, οι αζολες, απαγόρευση κατανάλωσης γκρειπφρουτ και προσοχη στη συγχορήγηση με κορτικοειδή (μεταξύ πολλών άλλων λιγότερο χρησιμοποιούμενων φαρμάκων). Οπότε λοιπόν προσοχή.. Σύμφωνα με το Medscape αν πρέπει ο ασθενής οπωσδήποτε να πάρει κολχικίνη και δεν μπορούμε να διακόψουμε τα φάρμακα με τα οποία υπάρχει αντένδειξη συγχορήγησης, μπορούμε να δώσουμε την κολχικίνη σε μικρότερη δόση ή με πιο αραιά μεσοδιαστήματα.

Ίσως... Δεν νομίζω όμως να είναι αυτό γιατί το να έχει κάποιος θετικούς δείκτες δεν σημαίνει υποχρεωτικά ηπατική δυσλειτουργία.

Ισραήλ: Αποτελεσματικότητα 99,96% με δύο δόσεις του εμβολίου Pfizer-BioNTech κατά του κορονοϊού.

Σχεδόν το σύνολο των κρουσμάτων ασθένειας Covid-19 προλαμβάνονται μετά τη λήψη δύο δόσεων του εμβολίου των Pfizer-BioNTech, σύμφωνα με έρευνα, με βάση τα δεδομένα από τον μαζικό εμβολιασμό των πολιτών στο Ισραήλ.

Η χώρα με τα μεγαλύτερα ποσοστά εμβολιασμού του πληθυσμού της στον κόσμο έχει καταγράψει νόσηση σε μόλις 0,04% όσων έχουν λάβει και τις δύο δόσεις του εμβολίου.

Από τις 700.000 πολίτες και μόνιμους κατοίκους του εβραϊκού κράτους που έχουν εμβολιαστεί και με τις δύο δόσεις, μόλις 300 μολύνθηκαν από τον κορονοϊό στη συνέχεια και μόλις 16 χρειάστηκαν νοσηλεία, σύμφωνα με τα προκαταρκτικά στοιχεία που ανακοίνωσαν αξιωματούχοι του χώρου της υγείας του Ισραήλ.

Το εμβόλιο, σύμφωνα με την έρευνα, έχει προστατευτική δράση και κατά της βρετανικής παραλλαγής του κορονοϊού.

Ο Ισραηλινός καθηγητής Eyal Leshem, ειδικός σε ζητήματα λοιμωδών νοσημάτων, σημείωσε ότι τα ευρήματα είναι "εξαιρετικά καλά νέα". Όπως είπε, "τα πρώτα στοιχεία είναι σχεδόν απίστευτα".

Η πλειονότητα όσων έχουν εμβολιαστεί με δύο δόσεις είναι άνω των 60 ετών, γεγονός που κάνει τα αποτελέσματα ακόμη πιο αξιοσημείωτα, πρόσθεσε.

Τα πρώτα στοιχεία δείχνουν σημαντική μείωση κρουσμάτων και μεταξύ εκείνων που έχουν λάβει μόνον μία δόση.

Convalescent Plasma: Tightening the Evidence About Antibody Titers and Early Administration

Convalescent plasma lowered mortality only if preparations had high anti–SARS-CoV-2 titers and were given before patients needed mechanical ventilation.

The potential benefit of convalescent plasma for treating patients with COVID-19 has only been shown recently, when a report indicated that plasma with higher anti–SARS-CoV-2 antibody titers administered early in the disease course lowered risk for death from COVID-19 (NEJM JW Infect Dis Jan 12 2021 [e-pub] and N Engl J Med 2021 Jan 6; [e-pub]). Now, data from a large retrospective cohort study support this strategy. Researchers analyzed the influence by titer of anti–SARS-CoV-2 IgG antibodies in convalescent plasma given to 3082 patients at 680 U.S. acute care facilities during the first wave of the COVID-19 pandemic.

Thirty-day day mortality was 29.6% in the low-titer group (titer percentile <20%), 27.4% in the medium-titer group (20%–80%), and 22.3% in the high-titer group (>80%). Relative risk for death from COVID-19 in the high-titer versus low-titer group was 0.75. In the subgroup of patients not receiving mechanical ventilation before transfusion, mortality within 30 days was 22.2% (low titer), 19.4% (medium titer), and 14.2% (high titer), whereas among those already receiving mechanical ventilation at plasma transfusion, mortality was 43.7%, 41.6%, and 40.5%. In adjusted analysis, the mortality reduction among high-titer compared with low-titer plasma recipients was demonstrated only in those not receiving mechanical ventilation (relative risk, 0.66 vs. 1.02).

COMMENT
While the role of convalescent plasma therapy in managing COVID-19 remains unclear, these findings indicate that any efficacy will be seen only if the plasma preparations have high titers of anti–SARS-CoV-2 antibodies and are given early in the disease course (i.e., before the patient needs mechanical ventilation). Editorialists emphasize that this regimen must be strictly observed, given the short supply of appropriately collected plasma.

For SARS-CoV-2 Detection, Saliva Performed as Well as Nasopharyngeal Swabs

Systemic review and meta-analysis showed that NAAT had >80% sensitivity for detecting SARS-CoV-2 in saliva samples.

Assessing nasopharyngeal (NP) swabs with nucleic acid amplification testing (NAAT) is considered the gold standard for detecting SARS-CoV-2, but proper sample collection requires trained personnel and is uncomfortable. Saliva, by contrast, is easily self-collected. In a meta-analysis and systemic review, researchers evaluated NAAT testing of saliva versus NP swab specimens. Sixteen studies were included; of these, 10 exclusively tested outpatients and 2 involved population screenings in which participants were not required to show symptoms of COVID-19.

In all, 5922 subjects underwent NAAT testing of saliva and NP swabs; 4981 had negative tests for both samples and 941 had positive tests for one or both samples. The pooled sensitivity and specificity was 83.2% and 99.2%, respectively (saliva), and 84.8% and 98.9% (NP swab). Similar results were seen in a sensitivity analysis limited to outpatients undergoing a single test with each assay.

COMMENT
Routine serial screening for SARS-CoV-2 is central to a comprehensive strategy for controlling the pandemic in universities, K-12 schools, and other settings where social distancing is difficult to achieve. This study provides evidence that using easy-to-collect saliva specimens rather than NP swabs will not significantly compromise such efforts.

Allergic Reactions to the Pfizer-BioNTech and Moderna COVID-19 Vaccines: Early Reports

The CDC's reports in late December 2020 and January 2021 indicated rates of 11.1 and 2.5 anaphylaxis cases per million vaccine doses of the Pfizer-BioNTech and Moderna vaccines, respectively, with no fatalities.

An initial CDC report detailed adverse reactions (including anaphylaxis) during the first 10 days of administration of the Pfizer-BioNTech COVID-19 vaccine. During that period (December 14–23, 2020), 1,893,360 first doses of vaccine (encompassing multiple lots) were given and 4393 adverse reactions (0.2%) were reported. Twenty-one of these cases were subsequently defined as anaphylaxis (median age, 40; 90% women), for an incidence of 11.1 per million vaccinations. Median onset of anaphylaxis was 13 minutes (range, 2–150 minutes), with 71% of reactions occurring within 15 minutes (the recommended observation period), 14% within 15–30 minutes, and an additional 14% after 30 minutes. Epinephrine (primarily intramuscular) was administered and no deaths were reported. Eighty-one percent of cases had a documented history of allergies and 33% had experienced anaphylaxis before. Other reactions included pruritis, scratchy throat, mild respiratory symptoms, and rash.

For the Moderna vaccine (released several weeks later), assessment of allergic reactions after administration of 4,041,396 first doses revealed 10 cases meeting criteria for anaphylaxis, yielding a rate of 2.5 such incidents per million vaccinations. Of the patients experiencing this severe reaction, 9 had a previous history of allergies (5 involving anaphylaxis). Female recipients predominated and multiple lots were represented. Nonanaphylactic allergic reactions were similar to those seen with the Pfizer-BioNTech vaccine. Most anaphylaxis occurred within 15 minutes, but one episode occurred at 30 minutes. No deaths occurred.

COMMENT
Although life-threatening anaphylaxis is rare, awareness of its possibility (and availability of epinephrine) is critical, especially as vaccination is extended to nonhospital sites. Because some of these anaphylactic reactions occurred beyond 15 minutes, observing vaccine recipients for at least 30 minutes is judicious.
